The forms below are what you need to fill out prior to having the RRSP issued by an Employer. Once you have completed the documents, you must submit them to the Canadian Office Plan Administrator in order to be processed. Once this step is completed, you will be issued a certificate number that you will need for the Employer forms. If you want more information, you can also go to their website. Other Documents, Policies, and Forms shall only be available to Plan Members in accordance with the Privacy, Communication and Disclosure Policy of Plan. Requests shall be made to either the Local office or the Plan Administrator.
